Avalere & the SCAN Foundation Release Person-Centered Care Roadmap

On October 29, Avalere Health and The SCAN Foundation released a roadmap on person-centered care sustainability, offering organizations specific examples of person-centered care models leading towards short and long-term savings.

These models, focused specifically on the needs and preferences of older adults with functional limitations and chronic care conditions, provide not only a guide toward financial readiness, but also toward improved patient care coordination. 

According to Avalere and The SCAN Foundation, a successful person-centered care model not only uses a targeted approach to identify the most appropriate individuals for participation, but also utilizes a coordinated care team to support and pinpoint the services of each person’s varying healthcare needs. This release urges organizations with programs already in place to improve the quality of care and the utilization of healthcare services to undertake a sustainability analysis.
