These new regulations stress the importance of high-quality care coordination for individuals who are transitioning across care settings, requiring providers to document nutrition interventions for those who are at high risk. Additionally, CMS has made a request for information on new measures that focus on addressing nutritional status, stating the connection between nutrition and outcomes.
