To gather this information, Avalere and AHHQI conducted literature reviews, interviews with policy leaders, interviews and case studies with innovative home health providers, and a review of the Institute of Medicine’s workshop on the future of home healthcare.

What’s caused the shift towards home-based care?

Over the years, there’s been a shift in demographics, patient preference, and a call for alignment to the Triple Aim goals. CMS/CMMI initiatives have also been created to incent the use of home healthcare in Medicare. Similarly, payers and providers engaged in alternative payment models are developing a strategic emphasis on shifting the site of care towards the home.

What did we learn?

Our interviews with policy and thought leaders uncovered their perspectives on payment and delivery reform and the future of home health.

The Framework for the Future of Home Health Care

Going forward, home healthcare will need to be patient and person centered, seamlessly connected and coordinated, high quality, and technology enabled. In the future, Avalere and AHHQI see home health agencies partnering with these entities in the following ways:

• Post-Acute Care and Acute Care: Provide services (e.g., skilled clinical care, care coordination, health education) to support the patient’s safe care transition to the home and community.
• Primary Care: Collaborate with primary care medical homes and home-based primary care programs during time-limited episodes where care recipients need an escalation in home-based care.
• Home-Based Long-Term Care: Work with home-based long term care and social support models (e.g., formal and informal person care providers) with responsive skilled nursing, therapy, and related services during episodes where care recipients need a brief escalation of home-based care.
