Avalere Partners with Industry Leaders to Address Hypoglycemia in Older Adults

Avalere collaborates with the Endocrine Society and Pottstown Medical Specialists to launch a quality improvement intervention study aimed at addressing care gaps in the identification and management of hypoglycemia in older adults with type 2 diabetes.On August 7, the Endocrine Society announced the launch of a first-of-its-kind quality initiative in primary care practice, HypoPrevent, which focuses on improving the prevention and care of hypoglycemia in adults with type 2 diabetes. Hypoglycemia (low blood sugar) can lead to seizures, coma, and even death. The latest figures also show that this condition has resulted in close to 300,000 emergency room visits of adult patients with type 1 or type 2 diabetes.
The Endocrine Society is working with Avalere and Pottstown Medical Specialists, a physician-owned multispecialty group, to create a 2-pronged intervention in their primary care practices. The first component of the project is a screening tool that identifies older individuals with diabetes who are at risk of treatment-related hypoglycemia. The second component is a clinical decision support tool that facilitates shared decision making with patients on ways to reduce their risk of hypoglycemia through the creation of individualized blood sugar goals or changes in medication management.
